The information supplied in this registration form will be stored by MMC according to our Data Protection and Privacy Policies. MMC admin staff will have access to this information, and the group tutors will have access to the bill-payer's email addresses for their groups.

We would like to add the bill-payer's email to our mailing list used to send out important announcements regarding MMC dates and events. This is optional, and you can unsubscribe from this mailing list at any time, but we strongly recommend this so we can communicate with you in 'all member' emails. From time to time MMC may record parts of activites by using photographs and videos of rehearsals and performances. These help us promote our activities. If you are happy for MMC to use photos and recordings in this way, please confirm this below (tick the box). If not, please ensure the box is not ticked.
Two Week Trial - If you are new to MMC and are unsure about whether to join then please complete this registration form and try out the course for the first two weeks, and then make your payment when you have decided that you wish to continue. Payment is required after attending more than two sessions.

As a charity, MMC is able to apply Gift Aid to termly membership fees. Gift Aid may be claimed on all groups (adults and children) so long as no personal tuition is provided (eg. Beginner groups or individual lessons). The Business Team will continue to monitor this and update this form should it change.Please note that if you pay a subscription on behalf of another adult, we cannot claim Gift Aid on this.Higher rate tax payers can obtain further tax relief by including the details on their tax return.
